This site contains all of the paperwork you will need to complete for the Department of Student Life prior to attending Providence. Please read through the information carefully! If you have questions about completing these forms, please contact Justin Bleeker, Director of Student Activities.

  • Student Handbook — This document includes the philosophy of the Department of Student Life as well as specific college policies regarding a variety of issues for students.

  • NEW! Student Insurance — Below is a link to Providence Christian College's medical and liability insurance website. Please review this website and do one of the following:

    1) If you have your own personal/family medical insurance coverage, please complete the online waiver form. You will need to have your personal insurance information (policy number, etc.) available when completing this form.
    2) If you do not have personal medical insurance, you need to complete the online enrollment form. We also recommend that you review the brochure to learn what benefits this insurance provides you. You will be covered August 15, 2010 through August 14, 2011.

    Anyone who does NOT complete the waiver form by August 24, 2010 by 8:59pm, will be billed $300 on their student account. (NOTE: The $300 charge will show on you student account on Populi until you waive the insurance.)

  • Final Transcript Request Form — (Optional) If you have not already sent in your final transcript from your high school, please use this form to request it from your high school.
     

  • Airport Ride Request Form — (Optional) Complete this form if you need a ride from the airport to campus.

  • Immunization Record Form — (Required) Providence requires all students to be in compliance with the California Department of Health Services recommendations regarding immunizations. These recommendations are also consistent with the U.S. Public Health Service Advisory Committee on Immunization Practices (ACIP), the American Academy of Pediatrics (AAP), and the American College Health Association (ACHA). All students need to provide medical proof of immunizations. Exemptions to the requirements are allowed for medical reasons or personal beliefs. (See the Immunization Waiver Form below. Please read this document for the list of required vaccinations to attend Providence.

  • Immunization Waiver Form — (Optional) Complete this form if you refuse to receive immunizations.

  • Medical History Form — (Required) For the safety of the student and the entire college community, each student must complete a Medical History and Health Form that will be kept confidentially in the office of student life. As a condition of enrollment, all students must provide evidence that they have health insurance coverage. This coverage can be through private (independent) plans or family (dependent) plans. This is the responsibility of each student. If a student does not have private or family health insurance, they can request a list of student health insurance providers from the office of student life. If you do not currently have health insurance, you will be required to purchase insurance through the college. (Please send a copy of your insurance card as well.)

    This form will help us care for the student in an informed way, especially in the case of an emergency. The information will only be used to ensure the health/safety of the student or college community.

  • Minor Medical Release Form — (Required) Complete this form if you will NOT be 18 years old before the semester begins.)

  • Vehicle Registration Form — (Optional) This form must be completed if you plan to have a vehicle at the Providence campus (resident or commuter).  You must submit it to the WCIU property services office found in the Aylward Hall complex.  They will then give you a parking sticker and access to the secured parking lot.
